Alcohol Health Home

The Alcohol Health Home (AHH) will provide comprehensive care management and coordination services to Medicaid beneficiaries with an alcohol use disorder. NMRE’s AHH model is comprised of a partnership between a Lead Entity (LE) and Health Home Partners (HHPs) that can best serve the needs of each unique beneficiary.

Your AHH Care Team will provide integrated, person-centered, and comprehensive care to successfully address the complexity of physical and behavioral health conditions. Individuals eligible for alcohol health home services are eligible for all Medicaid services. Alcohol health home services were designed to help beneficiaries connect to medically necessary services.
